YOUR INFORMATION IN GOOD HANDS
DATA PROTECTION (JERSEY) LAW 2018 (THE DPJL)
So that we can fully comply with the Data Protection (Jersey) Law we are requesting your consent for us to communicate with you for the purposes of continuing to provide you with optimum healthcare.
In order to gain valid consent we are asking all of our patients (aged 16 and over) that attend the practice for treatment, consultations or as a new patient to sign our GDPR Consent form in order to opt-in and update their profile.
You can withdraw your consent at any time by email to info@colomberieclinic.com or by calling 01534 727040.
Please be aware that registered patients cannot have their data deleted as it is necessary for the practice to retain clinical records according to the latest record retention regulations but they do have all of the other data rights. Please be aware that we are still required to continue contacting you with appointment reminders and recalls as they are an essential part of your patient care pathway but you can still tell us how you would prefer this to be done (email, phone, text and post).
Your personal information will never be passed to third parties unless you consent to us making a professional referral for you. If we have your consent for referral to another healthcare provider we will only send them the information that they need in order to provide you with the necessary assessment, tests or treatment.
Further details about how we process your personal information can be found in our Privacy Notice and provided to you on request.
